I want to gain weight in all parts of my body, not just increase my gut. I have super skinny arms and legs.

What should I do?

Check out Dr Stuart McGill for your back issues. Start there.

I would start you on a bodyweight program for 6~12wks. McGill's Big 3 + Convict Conditioning + Couch to 5k.

Then a beginner barbell program for six months. StrongLifts.

Then a bulk for six weeks.

That's a cautious route.
