You have to quit cold turkey. Never spend time alone in your bedroom; from now on it is for sleeping and sleeping only. Get a hobby and commitments that will keep you out of the house, and if you want to read, go on the computer or just lie down, do it in your living room where other people can see you. Also, never take your phone to the toilet.
The first two to three weeks are the hardest, after that the anxiety will wear off and it will be way easier to abstain from fapping. If you last for 90 days, your brain will rewire its reward mechanisms and you won't be addicted to porn or masturbation anymore. After that point you will be able to enjoy a fap every once in a while without it becoming a several hours long binge-masturbation session. Just remember to stick to your imagination only.

You should avoid masturbating to anything hotter than the average sexual encounter you would expect to experience.
