I have a word document with each exercise in a muscle group category, and I just do splits. For example, I'm doing Chest/Back today, so I'll check last weeks routine, and see what I did poorly in, throw that in, and what did I do the best in, if I've been doing well on an exercise really well and made improvements for a month, I change a variable for it (instead of doing bench press with a bar, I will switch to dumbbells, if I'm using the pec dec machine, I will switch to lying chest flyes, etc). There's never any confusion because I keep a clipboard with templates for where to write down exercises, reps and weight as well as other things. With all that considered, you should be able to make yourself routines specific for your body. It's all about trial and error, my friend, you find what your body needs work on, and what helps your body work on it. That's what's worked for me. :)
