Scooby's Rotisserie Core workout is probably the best place to start. Because you're so heavy, planks will actually promote a good amount of growth.

I've always worn mine right around my gut, since that's where I see everyone else wearing it (CWS being an especially offensive example, he wears his like he's trying to hold his tummy up). Tried wearing it high recently though, riiiight underneath my ribcage, maybe an inch below my sternum, and it made an enormous difference, my bracing felt SO much more effective.

Are you front squatting or highbar squatting? Front squat will do jack all for your hamstrings and glutes. Highbar hits them a bit.

Your knees hurt because you're not shoving them out(so they probably cave inwards a bit), which is also why you feel no glute activation. Grip the floor with your big toe, pinky toe, and heel, and twist the floor apart.

I think it depends on the lift you are doing and how you are built. I don't know who I heard this from but essentially the more perpendicular your torso is to the ground the further up you want to wear your belt. So a front squat you would want it pretty low and a conv Deadlift you would want it pretty high
