Where do I go if I want custom clothes, and to what cost
I've been recommended many designers but none quite have what I want

Bumping for a similar question to OP's. I really love some leather (women's) boots designed by Rick and Ann. D, for example, but they don't even come close to my shoe size (about a UK12, I think). While I'd like to imagine if I could come up with the money to purchase designer shoes at retail price, that they might be nice enough to make a custom size, I really doubt they actually would.
So, with that in mind, would it be possible to find a cordwainer that could make a clone that fits me? (And, hopefully without having to spend more than $500 for a pair of boots...)

Sadly im not familiar with the North-American market. If you want pure bespoke, France, Italy and Great-Britain, is where it is at. Everything Custom, shoes, suites, polo's, t-shirts, dresses you name it, however you will be paying for labour and material costs and often at top dollar prices.

45 = US13, which iirc is a UK 12?
I wear a 44, and I am a US 12.
