You could be ruining your skins natural barrier. This can happen when you use harsh cleansers, especially bars of soap. I would switch to a gentler cleanser. I use Cetaphil Gentle Skin Cleanser and just rub it on with my hands, there should be other cleansers that work the same.

A BHA would be good to prevent future breakouts, I like Stridex (in the red box) which has 2% Salicylic Acid.

If you work out, shower regularly, change your shirt everyday, change your pillow cases/bed sheets.
