Electrical tape. Lots of it. Wind it very tightly. Cover a good 4 inches of the cord so it won't break again.

Electrical tape is not suited for exposed applications. It gets ragged, sticky, and makes an unpleasant mess. Instead, use a self-fusing rubber splicing tape like 3M's 2242.
Take apart pen. Stretch spring a little bit. Wrap spring around cable. Either use fitting shrinking tube or, if necessary, wrap with electrical tape until minimal diameter for shrinking tube (that has to fit over the plug) is reached.

Is the underlying cable still in tact with no breakage? If so, the end of that connector looks pretty small and you should slip an inch or two of heatshrink tube over it and heat it tight with a lighter. That should stop any further issues.

I use liquid electrical tape for these.
This has very little to do with strain-relief. Apple's cords just..suck. I think it's the porous material. I'm going to try clear coating my next one and see if it improves the longevity.
