Different person, but if it hurts that much I'd recommend going up a size. When I started any rings smaller than 50mm would cause dull pain underneath the scrotum for me too. No, you can't add to your time if it's at the max. The only way to ever exceed your max time is if you still have requirement links or played games you need to go through when the time is up.
