First thing that happens is a mystical force selects people, seemingly at random, however as time goes one it's discovered that the selection was based on genetics and personality as well as attributes including physical.
These people, representing 3-5% of the population, will later be called "Nobles." If they aren't already, they become gorgeous, recognizable as the same person, but otherwise more attractive. These people do not age, if they are over 30 they regress back to that age or close to it. However their fertility rate among themselves is lower, low enough that they keep their 3-5% proportion. These people are quickly seen as the future rulers, and separate the world into their own fiefdoms. These Nobles split into multiple, what will be called the new genders.
1.) Female, the typical female, however all are gorgeous and all have perfect proportions.
2.) Male, typical men. However their dicks are all roughly 7 inches long. If anyone who becomes a Nobel as a bigger dick, it shrinks.

Now for whatever reason, probably because of their connection as a direct descendant of a futa, E-males are treated as Nobles, but because they age, they are never actually one of them. They tend to fill the upper echelons of the world's new militaries. Being universally intelligent and very trusted by the Nobles, they were trusted early on with leading armies and being the right hand of the local ruling Noble. Now the Others generally accepted their lot as second class citzens, focusing their resentment on the Demis and this pushed many Demis into the direct service of the Nobles.
