Because of the way the stomach is positioned in the abdomen, it's almost impossible for the belly button to push out from over-eating irl. The uterus is lower so it can be pushed out during the late stages of pregnancy, though. I think a lot of artists extend the same logic to their drawings.

Use virtually any competent image manip tool that supports layers, paste internals on top of the external layer, crop out parts of it, soften the crop for smoothness, decrease the opacity of the internals layer to your desired level, certain color filters can help blend it in a little better as well.
