For me, "identity death" is more like a combination of brainwashing/hypnosis, and turning the subject into something other than themselves. Which, ironically, nobody seems to call each individual one identity death.

For example, a protag in a story is being bullied by a super strong angry jock, but gets his back by turning him into the exact opposite. A short, hourglass girl, and then hypnosis him into having a personality of a gentle, cutesy, and pacifistic girl with no memories other than loving the protag.
The only difference is that it's made obvious when the guy-turned-girl does things like peeing standing up or is suddenly able to lift things a small cute girl couldn't. Or remembering things suddenly that they shouldn't and wonder why.

In the end, the point I'm trying to get across is that "identity death" is about perspective.
To me, they are the same person, they are just changed from the outside, and forced to think a certain way from the inside. The soul or core is still in tact. IMO, of course. Take with a grain of salt, please
