Rape was pretty common after sieges due to the chaos it would cause and the amount of soldiers involved. Bands of soldiers acting on their own behalf and pillaging towns was (I imagine) also a bit frequent, but they would have been stamped out by the local lords unless there was much anarchy across the region.
Male on male rape in war happened I imagine, but I have not read sources on the matter. It was most likely something that the victims would not have reported because of the association with sodomy being punishable by death and... well... men not often reporting being raped.

To me the monster rape is an afterthought and the hot part is imagining your only protection being ripped away from you, and what it must feel like to be completely exposed and vulnerable in a hostile environment.
I guess I just really like exposure creating vulnerability and fear, and losing armor is a great way to do that.

Most warriors were armed peasant or mercenaries anyway and knights only represented a fraction of armies.
So yeah, you could expect quite a bit of rape going on with men spending weeks without their spouses.

Literally every time a city fell or they could get their hands of 'enemy' women. A city falling to seige just turned into an orgy of rape and violence until the attacking army was exhausted and/or too drunk to continue and their commanders could reestablish control. During the Hundred Years War, the English went of long distance terror raids called 'chevauchée' where they basically roamed the countryside, stealing everything not nailed down and burning everything that was. French peasants used to flee into the woods or hills at the mere suggestion the English were nearby, since if captured, the women would be raped and the men tortured to find where they'd buried their valuables.
Rape was, and still is in much of the world, used as a weapon against enemy civilian populations. Both male and female. So yeah. With the exception of monastic warriors like the Templars and Hospitallers that took chastity vows, codes of chivalry didn't really offer any protection to non-noble women, and regardless, the bulk of medieval armies weren't knights or true nobles, just professional soldiers fighting as mercenaries or retained men-at-arms.
