You can't be trying very hard. There's quite a bit. If you're cum soaked, and wash yourself off in the underground lake on the first floor, you can be attacked by the lake creature and filled.

Hell, right outside the starting room, you can sit on the throne and there's a chance it'll start filling you.

They added a graphic that follows the progression, but be prepared to read everything.
