I'd imagine with some training to your gag reflex, a much thinner one could possibly work. Though the sizes depicted above would probably be impossible. Couldn't extend past the back of the mouth though unless it was thin enough to still have room for air to go in by breathing with the nose I'd assume.

The thing is that the gag goes in your esophagus, so having a regular tube in its middle is not going to do you any good because air will not reach your lungs. The problem is that the gag is usually so thick that it blocks your trachea so you can't breathe. Maybe a very thin gag might be feasible.

It's not all that hard to breathe, so long as you don't gag too much you'd be surprised how much room is left in your esophagus to breath through your nose. That gag in particular doesn't go back far enough to trigger the gag reflex, but it does feel much bigger than it is for people who haven't used that type of gag.
I think the biggest difference compared to other gags of a similar size is the feeling of pressure you get when it is completely full.
