Horse show that must not be named did it a few times, the funny thing is it's always painfully obvious which characters are fan OC's, none of the look even remotely like they belong in the show.
Charcoal, a character created by fans for a Wizard contest to create a Thunderbolts character. Wizard were idiots though and failed to properly specify who would have legal ownership of the character, plus never followed through on other stuff the winner was supposed to get. So the fan attempted to establish legal ownership and when Nicieza killed Charcoal intending to bring him back Marvel told him not to bother and just keep him dead.
Also it's only vaguely /co/ but every robot master from Mega Man 2 through Mega Man 7 were done via fan contests. Mega Man 8 had six of the eight bosses as fan submissions with Tengu Man and Astro Man being the only ones that weren't.

They were mainly just side characters, Thunderlane was just a member of a crowd who had a handful of lines and Stellar was basically just selling something that one of the main characters wanted, most of them only got like a minute of actually being relevant to the episode.
