Golden Age/Man Who Laughs Joker = Joseph Kerr
Modern Age/Killing Joke Joker = Jack Napier
New 52 Joker = name unknown

Though I think the one depicted with the Man Who Laughs art was supposed to be Silver Age Joker, which would make more sense because Golden Age Joker is very similar to Snyder Joker and Silver Age Joker is nothing like any other version.
