Keeping things low poly is an admirable endeavor, but this is a bit too much. We aren't running N64's anymore and you need polys in key places so that things bend properly. Them arms and legs gonna fold into each other.

The solution on the right provides natural bends.
There's no way in hell that model will be able to do anything other than a static T-pose.

Almost exclusively yes. In the same way science that's advanced enough can seem like magic, you slap enough polygons on something to make it look smooth.
Alternatives that aren't widely used exist and can be seen in games like Cube World (voxels) or point cloud rendering (Dreams PS4)

The key difference between video game graphics now and video game graphics from the, say, PS2 era is that they use high definition textures with sophisticated rendering algorithms which convincingly create the appearance of far more detailed geometry than can be rendered in real-time using polygons. That and just plain more polygons, of course.
It's like how everything is still just a bunch of pixels on the screen, but we've come a long way since the NES.

Yes, but modern games are powerful enough to use a bunch of techniques to make them look better than older models. Things like bump mapping can completely eliminate the "polygonal" look of a model when used well.
