That depends on if you're talking about the manga or anime, and your definition of lose.
In the anime, Chaos Galaxia kicked her ass, but that was Chaos augmented by the power of Sailor Galaxia, and anime Sailor Moon isn't as strong as manga Sailor Moon.
In the Manga she threw Chaos into the Galaxy Cauldron, the source of all "Stars" (Stars being both literal, and a metaphor for souls) in the universe. But like Usagi herself, Chaos would eventually come back from that and they only way to prevent that was to destroy the Galaxy Cauldron, the preventing new "Stars" from ever being created. Usagi was unwilling to do this, so technically Chaos will come back one day, but right now he is bust not existing.

This is something the English dub actually handled better. Serena starts off as a huge fan of Sailor V and tries to model herself after her in the first episode, which does a much better job of explaining why she acts the way and holds the ideals she does while trying to fight despite being a clutz otherwise.

Toei's already done crossover episodes with DBZ and the only thing they really have in common is fighting bad people, and MC's with a love of food (which Sailor Moon also shares).

The styles didn't really mesh well, especially in terms of the ladies' figures, and it COMPLETELY ignored key points of each universe to mesh them into one weird planet they all existed on at the same time.

But the character interactions were interesting, it was harmless fun, and it can be the same with a Sailor Moon crossover. And with DBS venturing into multiverses, it gives them an easy opportunity for the two paths to cross for even a short time.
