Ren and Stimpy, I guess. Not sure if that's the first cartoon to do them like this, but Spongebob (especially the early seasons) definitely takes some inspiration from that show.

Heavy inspiration from Ren&Stimpy.

In my opinion, the show that captured grotesque closeups better than any other was probably the Misadventures of Flapjacks. Just because it served as a constant reminder that we are seeing Stormalong through Flapjack's eyes, and that beneath the veil is a grim reality of a child in negligent foster care in a crime riddled town. It served a purpose.
