You also need the launch codes from the president, which is authorization. If he delegated the actual launch to someone else, this scenario could happen.

they already had the authorization to use the nukes and the general had the nautilus on standby with the nuke. on the radio only he had, and mansley snatched it and just said "LAUNCH NOW" so they heard launch and went with it. you could suspend disbelief and rationalize it 5 ways from wednsday about radio distortion, high tensions, etc etc. This IS cold war America after all, hair triggers apply

Not personally, there's a very detailed complicated list of the situations of when and when not to launch a nuclear retaliation or attack. The whole point is that these professionals will be able to act efficiently when the order to act comes.
