He didn't actually know they were behind him at that moment, and in fact they weren't, they hadn't made it out of the cave yet during this scene. This was just the first sign Jack's mental state and injuires were coming together to distort what he was seeing. It was a foreshadowing to what we would witness in the cave.

You know, proper hints to setup for a bigger event that would occur shortly.
