I'm just wondering how Moonflower program ended up in such a sad dysfunctional state.

Moonflowers are provided with all the equipment necessary for defending from Big Bad, but Little Dreamer rarely helps them, besides the usual supply drops. And no one tells them what to do - the only bit of advice Hanazuki ever got was "grow" in the first episode. It's sheer luck she found someone who knew what to do. The others weren't so lucky, and it got (at least) two moons destroyed.

Some Moonflowers are just defective. Kiazuki is a sociopath with no way to activate treasures - quality control is nonexistent. No one ever checked her before or after deployment, and her moon never had a chance.

Were Moonflowers always like that? Expendable defense forces with low success rate, deployed to the moons without any supervision? Or something happened to creators of Moonflowers, and now there is no one left to keep them in check?

I'm not certain Kiazuki was defective or just crushed by failure. But Hanazuki did indeed get lucky.
Little dreamer is supposed to be the quality control but seems to do very little. I say "seems" because he's done some stuff under the radar so far that leads me to believe he wants to fix the trouble with the moons.
He pays special attention to Hana from occasionally leading her to the answer to something she asked to straight out giving her a large stash of treasures when she was in trouble. I also think he might have some foresight ability if not omniscience since in episode 6 he sticks around Hana's moon until Hana creates the rainbow treasure. That treasure is special and I'm willing to bet it can revive a moon and that that's why Little D pays special attention to Hana.
