His lungs are already long gone; he uses a pymaric device to speak, same with his eyes. I don't know how intact his other sensory organs are or whether brain-rot has any effect, but he does slow his rot using herbs and, if I'm remembering right, by core-leeching bacteria every morning.

Pretty sure all his organs are gone, including his brain. Thinking and hearing are functions of the soul, which is why he can still do them. In fact, I think Ashley said once that he essentially has a perfectly photographic memory now that his mind operates separately from his body.
