Well, that's assuming she would have had the surgery after entering the comic. We can assume she had some kind of life before then, and when Marten and the audience meet her, she already identifies as female, and we don't know how far along she is in her transition.

Also Jeph did hit us with that vague timeskip, the one where a couple characters got new hairstyles, so we don't know what may have happened off panel then or quite how long it was supposed to be, and the whole timeline is generally pretty fucked all over, and plenty of characters including Claire drop in and out of relevance in the comic for odd stretches.

Still, all of the above are kind of flimsy excuses and just going by statistics and her age, I think it's unlikely Claire has had surgery.
