I don't think he was ever human. His mother and father were some sort of evil spirits as he tells it. He says they pressured him to become a Grim Reaper when all he wanted to be was a wood nymph or a satyr or some such.

imo it's a fun subversion of the "ancient deities/demons or whatever all have English accented solemn voices" trope.

Originally he was supposed to be British but Maxwell Atoms didn't like how he sounded.

The VA the decided to reread his lines with a Jamaican accent, akin to old Jamaican voodoo witch doctor stereotype. Atoms loved the new voice and not only decided to stick with it but did some rewrites to better fit the new accent "The "Limbo" gag in the pilot, his habit to refer people to "man" after the end of a sentence and etc.).
