Lady Shiva appearing in Hush for like six pages turned out to be a fucking coffin for her. She said she was doing a favor for the League of Assassins bringing Talia back, but because Hush is such a huge entry-level book, that ended up being a lot of people's first impression of her, that she's a strong evil kung fu chick who works for the LoA.
And now, almost 15 years later, nearly every single Shiva story has her working for the League, either by choice or by obligation, and they all suck major balls. There is seriously not a single good Shiva/LoA story. And the fact that cross-media adaptations like Beware the Batman and Arkham Origins perpetuate this only add more nails to the coffin.

Question's problem to me goes a bit further than JLU. It's a nasty mix of people who want to write Ditko's Question (Miller, Morrison), people who want to write Rorschach and use Question as a stand-in (remember that his first New52 appearance had him straight-up using Rorschach's word balloons), people who can't figure out how to make him work in a superhero universe, and an overall line climate that's veering more and more away from long, low-key character development. In the quagmire of all that, O'Neil's Question sadly gets lost. Nobody wants to use him because it's easier to ape Ditko or Moore, and also because to use him to his full extent you need a long run, a low-key setting, minimal interference, all things modern mainstream comics are sorely lacking. The only other writer who loved O'Neil's Question enough to make it work was Rucka with Cry for Blood and 52. He *really* loves O'Neil's run.

Then, once they had basically reduced Guy to a mindless shell alive in body but not in consciousness, Sinestro used Guy as a remote control puppet to try and kill Hal, who upon hearing that Guy was dead, started fucking his girlfriend and would have married her had Sinestro not used the braindead Guy to attack Hal.

Guy was put into a nursing home, to spend the rest of his life as a vegetable, until Crisis happened and the Guardians finally got around to finding a use for Guy to warrant undoing what Zod and Sinestro did to him.

They fixed his brain to restore his mind the best they could, but the end result was Guy having ZERO filter (think Sophia on Golden Girls) on what he said or thought or acted. Plus they had to exploit how Hal and John repeatedly fucked Guy over from his destiny to be a Green Lantern/his desire for revenge against the two, just to give him a reason to live as opposed to staying in a state of catatonia.

Basically the universe fucked Guy Gardner over 10 times over, to the point that Hal/John basically have to eat shit and tolerate him because he was having a shitty hand given to him by life.
