I was fine watching this episode as a kid... except for one part...
When Dib is trying to be brought back by the medics and Zim replaces the paddles with piggies. Then Dib starts flatlining, confirming that he's dead.

And Zim just has this victorious smirk on his face while the flatline can be heard in the background, and simply takes a victory sip out of his drink.

THAT part was pretty messed up to me.
