I think the first noises are him getting out, and he hits her/shy cries out and she's gone. If you have a crushed brain - even if you're still technically alive - chances you aren't doing much. And her regen isn't instant, and might not work on the brain at all. Though I'd argue the mosak's powers are provably not based in physical biology from what we've seen so far.
