Captain Mar-Vell was an actual Captain in the Kree military, and some idiot misheard him when he said his name, so people kept calling him "Captain Marvel", until he embraced the title, and most of his legacy heroes took his title.
Captain America, depending on the version, actually WAS a military Captain at some point.
