He saved her at the ice palace to try and convince her to end the winter. He finds out she doesn't know how so he "officially" sentences her to death.

When were the other two times?

When Anna asked him to kiss her, he had the chance to show what kind of a man he was. He decided to show that he's a callous prick who'd smugly taunt a dying woman, and he revealed his plans to overthrow the queen. Up until then he had done pretty noble things out of selfish motivations, but in that moment he showed that he's not a very decent man.

Sure, the conflict originated from Elsa. She did some stupid shit during the movie and showed a lot worse leadership skills than Hans did. It's totally possible that Hans could've been a better king. The reason why Elsa is a good guy and Hans the bad guy is mostly between their intentions. Elsa was thinking of her sister and trying to keep her safe, but ended up hurting her without meaning to. Meanwhile, Hans' actions appeared kind-hearted and beneficial, but he was just coldly manipulating people (and willing to kill them) in an attempt to advance his own status.

The movie had a scene where Hans simply asked Elsa to stop the snow. He didn't know if killing Elsa might end the magic winter, or if it would mean taking out the only person who can fix the situation. That's why he was hoping that keeping Elsa alive and asking her to end the snowfall would be the easiest way to solve the problem.
