As the others have stated, he included living people in his delusion. He might have thought he was right but making a guy drive around in an ice cream truck for 40 years is a bit fucked up.

I feel sorry for thse people mst of all in projected fantasy worlds, they usually know what's going on and are in the wirst loops of them all. The perfect neighbors or people around the community are given a routine but characters like an ice cream truck driver or a person who rides by in a bike and waves literally drive around town in a circle until it's dark and then just start up again the next day. That seems like worse former than just being stuck in a house pretending everything is all right.
