Most places it is assumed but out east or at Neopolitan places you can't assume you're getting cheese so I would come down on the side of "topping" as it's not an essential ingredient

It has to have cheese in order for it to be considered a pizza otherwise it's just saucy bread. It doesn't have to be a whole layer like American style pizzas but some is required.

Why do people talk about cheese pizza to refer to child porn? I mean, it's not like cheese is fresh or anything. If it was a nice ripe fruit fresh for the plucking then I could understand, but it makes no sense.
