I hate when I'm at a restaurant that serves burgers and they ask me how I want it cooked. First of all, at a sit-down restaurant that cooks to order, you can guarantee the patty will be too thick so the burger either comes out extremely dry if you order it well (as you should with ground meats) or a mushy mess on the inside if you order it medium, which is disgusting. It doesn't really matter if you're at a "nice place" or not because the foodborne illness risk is going to be about just as high anywhere that has an equivalent restaurant food safety rating. The issue to me is more the texture of the uncooked ground meat in the center of the patty. This is why burger joints are always going to serve better burgers and are closer to serving the burger ideal.
