The bubble form because many pre-formed frozen patties are dotted with tiny holes. During processing, freezing, thawing, and cooking, the outermost layers of meat can get a little roughed up and basically form a protein-rich meat goo that has enough surface tension when cooked to form defined bubbles.

It looks disgusting, but is not indicative of anything really wrong, besides using frozen pre-formed patties
