You had a lot of milk+bread or other filler in it.
It was not a lean beef.
You didn't glaze it.
You didn't cook it until the top was browned.

Slap on some ketchup or other glaze. Use a spoon to drain some of that fat. Put it back in the oven uncovered until glazed over.

Bread soaked in milk makes it more tender. Try meatballs, half with that and half without. Mind blowing difference.
