I'm only 5 hours in so I'm not sure, however, it has shown her with it off for a second during a cut scene. She woke up and didn't have it on, then stood up and slipped it on real quick.

I don't think so, a lot of the androids in the show have it, but there's also a bunch where they have something over only their nose and mouth and their eyes are revealed. I think it's more of uniform? [MINI SPOILER} Also, when she had her blindfold off in that screenshot she had just "died" kinda. Not really a spoiler because it happens very early in the game, basically she uploads her memories to the cloud I guess and her HQ remakes her body and then uploads her stuff to her or something. She had just awoken this in the screenshot.
