I usually convert the path into a selection, with 0,1-0,2 pixel feather depending on how big the picture is, and delete. Then I add a slight stroke to the result with the right type of blend to make up for the stroke that got cut out.

Also, seasonal transparent!

How long does one picture take?

Depends on how fast you are, being efficient with the pen tool takes practice. It also depends on how many curves the picture has, the Yui image I posted took a while because of the wool. I don't remember exactly how much but I'm guessing around 15-20 minutes. I put more work into it than most would though, since I want the result to be flawless.
