Why don't all banks and credit card companies require two-factor authentication (2FA) when withdrawing/depositing or making purchases at retailers?
Would this not curb credit fraud and theft or am I missing something?

It would require a massive infrastructure upgrade, not to mention that what you describe is fundamentally not how credit cards work. The significant of this being that only honest merchants would be held to 2FA.

Credit card and bank transactions are reversible. They would rather have the transactions be easy to make even if it means they have to spend some manpower on manually reversing fraud.
