You need two things, an EIN and a sales tax ID. They're both necessary to open accounts with wholesalers and file taxes.
Beyond that, you can file for a DBA (Doing Business As) but it's not super necessary if you use your name (Smith Plumbing) or stay small.

Not customers, not suppliers, not employees, not affiliates. They will all leave you holding the bag if it's convenient. If you're one of those types that think taxes and regulations are the only problems a business faces because some dropout on the radio told you so, you're in for a rude awakening.
You need to file forms with your state/locales department of commerce, possibly the city, its good to have separate bank accounts which requires the EIN.

there's no federal requirements except you must make a profit and pay taxes on it.
you don't need an EIN unless you employ people. You don't need a sales tax license unless you sell retail. You don't need an LLC and you don't need to register a DBA in most states.
so honestly the only requirement is that you turn a profit and have a business license.
