It's difficult because it takes time to bleed out, so you are usually found. Also people often don't hit the right veins or just go across, in which case it can clot. It's much more difficult to clot when is on a vein going down the length of the arm. People also usually are sitting up instead of laying down with your head below your feet. Basically it fails a lot because people are lazy about it.
