As far as how people react, you can never tell for sure ahead of time. Sometimes people you thought would be cool will be dicks about it, and sometimes you will get support from people you never expected. At some point you gotta put your needs above others prejudices though. I haven't had too much hate, but I live in a pretty liberal area too so that helps.
Figuring out what you want out of transition is pretty important, it might be worth talking with a therapist with relevant experience. I just sorta dove right in and educated myself about others experiences and stories, and found what I could identify with. Putting myself in their shoes helped a lot.
You don't even really have to have an end goal to start; I just started hormones, and adding femme stuff to my every day presentation and seeing what I liked. If I didn't like something, I stopped doing it. People tend to think you must move from box A to box B or whatever, I say fuck a box. Do whatever you want.

I would just call and ask to make sure they go by informed consent, and if they do just say you're trans, want to start hormones, and set up an appointment. Most places will have you come in the first time just to talk about your goals with hormones, your medical/mental health history, and take blood for baseline levels.
