The word "skim" in the context of milk means to remove fat from. The cream (containing most of the fat) rises to the top, where it is skimmed off for the purpose of removing fat. However, the cream layer reforms easily with enough fat. In order to actually skim away the cream layer, the fat has to be removed. If you want milk with low/no fat and it comes with a creamy layer that hasn't been skimmed, it's not fully skimmed milk is it?

Of course divergent linguistic evolution happens, but it should happen for sensible reasons.
