If you have problems staying concentrated for too long, the best you can do is set an appropiate space for working. Leave things around to keep you stimulated; jump from one topic to another if you think staying in one too long bores you; also, take regular (but not extensive or sinking) pauses to have breaks.

If, by the contrary, you just have something in the back of your head that's bothering you, go attend that issue. It'd be better to work on a cleaer head in the long run.

How can I come to terms that I do not have the self motivation to get my life together and it's nobody's fault but my own.

It takes self-awareness to recognize one's own mistakes and self-pity to dwell on them, but it is always better to improve. If you want to be productive with whatever has happened to you, accept the fact that you've screwed up and don't let it take your whole life down; profit this moment to rethink your priorities, establish realistic short term (to keep you rewarded) and long term (to enjoy later on) goals and find new, creative ways to live.

Motivation is an emotional factor and, like most emotions, it can fade away. Try building habits according to your values; they usually outlive and rekindle your willpower.
