Take a look at this screen shot and ask yourself how you would knee him in the face. His head is placed to the outside of your hips and to make matters worse he's driving into you making a forward moving strike wirh your legs damn near impossible.

The only realistic defense against a double leg is to sprawl

Literally had half a second to hit a target that was moving towards him after throwing himself off balance with a missed punch. It's not reasonable to say "why didn't he knee him in the head" when your margin of error is so small.

And even if you could nail that one in a million shot, you better hope it knocks him out because otherwise you're on one foot and he's got the other in his hands.

In fighting game terms a knee is a "prediction" counter whereas a sprawl is a "reaction" counter. There are many move pairs like this, and one of the main problems with many traditional martial arts is that they treat everything as a reaction counter because they do compliant drilling instead of sparring. In an actual fight you'll probably have a lot of trouble catching a punch or kick or landing a knee counter to a double leg unless your opponent has showed a habit or pattern.
