can't blame your mum for wanting to get you to grow up (depends a little on your age though)
also, I am pretty sure that they're not allowed to share your medical record with anyone just like that, if needed, make her sign something so that she won't tell your mum?

Because if you are a public disgrace to her like what you seem to be heading to, she will kill you with her bare hands.
If she doesnt want to help her kid, prepare for beeing homeless and look up what you can do then, maybe ask her to receive post at the old address, so you can start a job with that and not have to go through homeless programs for a post address.
