Oh man, I just did this exact thing last weekend. Was the first time meeting an online friend for me.

1) You're going to be fine. You will feel uncomfortable and awkward and then you will have a good time.
2) Tell her. Say something along the lines of "I'm nervous about meeting in person, I feel like I'm going to be awkward / embarrass myself / [insert whatever you're feeling here]". If you share that, then if it does end up that way, it's easier to shrug it off, laugh about it, etc. Assuming this person is not an asshole, they will understand.
3) Have any activities planned? Sometimes it helps to have an idea of what you're going to do together, so you have a mental out if just sitting and talking is a strain.
