So me and an ex of about 3 years ago started talking again recently after she initiated.
After a while of talking I asked if she wanted to go out for a drink to which she said yes.
It's a couple of days away, and now she's saying that while she does want to go, she's 'worried'.
What does this mean? What should I say?

She might be afraid that she'll grow feelings for you again, idk seems silly if it's been 3 years.

yeah dont sweat it, just see her as a possible prospect nothing more. Always have it in your head if this doesn't work out, then whatever you'll try again and find another there's no shortage of women. Unless you got the oneitis, in which case you need to deal with that immediately. That being said, the more cool you play it off, like take it or leave it kinda deal the more they're into you alot of the time. (Not always of course incase any femanons get triggered and are like "IM NOT LIKE OTHER GURLZ")
