It takes a while 9-12 minutes or so depending on how aroused you are. Yes it hurts, you can decrease skin stretching pain with lube but as it draws blood and fat into your dick, it also tugs real hard at surrounding veins, you also have to be careful and separate your balls from the pump or it'll tug at your balls hard as hell. You will need lube or at least soapy water to make sure you get good skin to tube contact. You don't need to achieve maximum results for a numbing effect however, that can probably be achieved in 2-3 minutes(?) Just enough time to draw fat into the glans.

I find that it helps to focus on your breathing during penetration. Maintain you're attention on the air going in and out of your nose, keep breathing no matter what. When you're about to cum the first thing you do it stop breathing, so if you don't do that it can hold it off. Another thing is try not to tense up the muscles in your lower abdomen, that's another thing your body does before ejaculating and if you don't do it it'll hold it off.
