For women, orgasms are about 75% mental at any given time. That's not to say pure manual stimulation can't overcome things sometimes, but usually if she thinks she can't orgasm, she won't orgasm. ESPECIALLY if she's got some sort of fear reaction attached to her orgasm. Assuming she's being on the level with you, she's going to be so worried about what might happen when she orgasms I imagine she doesn't really enjoy sexual interaction at all.
