How do I approach a psychiatrist for having a re-evaluation?
I was diagnosed with depression and ADHD as a teenager but I think those diagnoses were probably wrong and if that's the case, I'd like them overturned for career reasons.
Who knows about them at this point in your life?

It's on my medical/insurance records so if I were to lie about it (i.e. not check the boxes for depresion/ADHD on a medical form) I could get in trouble for perjury.
I'm still on my parents' insurance, if that changes anything.

Wait, even if you're re-evaluated, the question is "Have you ever been diagnosed..." and the answer would still be either yes or a lie. Nobody can get your juvenile medical records. They would need to subpoena them in court. In the event you had to submit them, redact anything before your 18th birthday as "nobody's fucking business."

I would start going to your doctor.

Are you in literally every fucking thread? Fuck off, doctor shill.

Well, yes i am everywhere obviously (like a god who sees everything) and when you have in your medical records written:
>crazy
and want to get rid of it, you try the simple solution first. You go to your normal doctor, tell him your situation and he will tell you your options.
Like in my medical records is that i am obese. But you see, i havent been to doctor in years so it isnt true anymore.
And sadly, your future employers have a wide range of tools to dig out your private life information. In usa your ISP will sell your internet history soon for example legally.

The question I have is if I should openly say "I have these diagnoses, and I'm not sure they were correct and want them re-evaluated," or if I should just say "I want an overall evaluation."

Use better formulation:
>a lot of time passed, i think i solved my problems and these medical records might pull me down when looking for job.
Just basic human comunication dude.

>I could get in trouble for perjury.
Only if you were under oath at the time. Is your career legal or something?

this. Any psychiatrist would re-evaluate you if you approached them from a rational point of view like this

Show me a lawyer who isn't a hell-bent psychopath, and I'll show you a penniless bum who just got left by his wife.
